For case managers, hospital discharge planners, and ADRC staff in Mesa County.
The short version
This is a free list of Mesa County assisted living homes that confirmed an opening this week.
If a home does not confirm within 7 days, we hide the opening and treat the home as full. The name can stay on the board. The opening does not.
A listing is not a reservation. Call the home.
The weekly rule
- A home is shown as open only after it confirms.
- That confirm is good for 7 days.
- If we do not hear from the home again in those 7 days, the opening is hidden.
- Hidden means assumed full, not deleted.
- We do not guess an opening back onto the list.
That is the point of this board. Other lists can leave old openings up. We do not.
How to filter
You can narrow by:
- City — Clifton, Grand Junction, Fruita, or Palisade
- Care type — unlocked ALR or secure environment (Colorado does not issue a “memory care license”)
- Pay source — private pay, ACF-EBD, or ACF-CMHS. ACF means Alternative Care Facility. That is a Medicaid services certification. The resident still pays room and board. Not every licensed home is an ACF.
- Single room
- Private bathroom
- Household — mixed, women, or men
- Pets okay
Phone numbers are on the cards. Nothing is behind a login.
If we do not have sourced data for a filter (for example ACF, room type, or household), that filter cannot invent a match. We will say so.
When there is no exact match
We may show a next-best opening, and we will say what changed.
We relax, in this order only:
- Adjacent Grand Valley town (Fruita, Grand Junction, Clifton, Palisade)
- Private bathroom
- Private / single room
We never relax on our own:
- Confirmed this week
- Unlocked ALR vs secure environment
- ACF, if you asked for it
- Two-person assist, if that is required
We do not stretch to Montrose or Delta to fill the page.
What to do with a result
Call the home. Ask if the opening is still there. Ask about care type, ACF, room, and bathroom in that call. Then use CDPHE Find and Compare for the licensed name.
This board does not choose a home for you.
